Where do I start?
Knowing what decisions should be
made and when can be extremely difficult for individuals faced
with the issues of an aging adult, be it a parent, a spouse, a
family member, or a friend. At E.I. Associates, LLC, we provide
you with the guidance that you need by completing a
Confidential and Comprehensive Geriatric Assessment (CCGA).
The CCGA is completed by a registered nurse (RN) and is the
first step in determining the needs, preferences and resources
of you and your family.
Based on the CCGA, we prepare a
comprehensive plan of care. The plan of care is tailored to your
individual needs and focuses on utilizing the resources
available to you and your family. We can provide as little or as
much care as needed. For instance, we can provide the following
services for you:
Arrange and monitor in-home services
Select and coordinate new living
arrangements
Act as a liaison for families at a
distance
Act as a financial or a medical
advocate
